The answer is I hate them. They are bastards and scum. They want death for us, for Russia. And while I'm alive, I will do everything to make them disappear.

For the record the different sources I can find disagree as to whether the “Them” in this post means Ukrainians, western liberals, Russian Liberals or all of the above, but the quote belongs here whichever.
(Also I’ve seen “scum” translated as “degenerates” and “geeks” in some sources as well.)

Medvedev probably deems them false Russians, as they do not manifest the Will of the People (read: authoritarian hive mind). Pretty clear marker of at least ur-fascism, according to Umberto Eco:
13. Ur-Fascism is based upon a selective populism, a qualitative populism, one might say. In a democracy, the citizens have individual rights, but the citizens in their entirety have a political impact only from a quantitative point of view – one follows the decisions of the majority. For Ur-Fascism, however, individuals as individuals have no rights, and the People is conceived as a quality, a monolithic entity expressing the Common Will. Since no large quantity of human beings can have a common will, the Leader pretends to be their interpreter. Because of its qualitative populism Ur-Fascism must be against "rotten" parliamentary governments. One of the first sentences uttered by Mussolini in the Italian parliament was "I could have transformed this deaf and gloomy place into a bivouac for my maniples" – "maniples" being a subdivision of the traditional Roman legion. As a matter of fact, he immediately found better housing for his maniples, but a little later he liquidated the parliament. Wherever a politician casts doubt on the legitimacy of a parliament because it no longer represents the Voice of the People, we can smell Ur-Fascism.

See, initially I thought they meant that they hate telegram users, and I almost related to that for a second.
Luckily, that feeling subsided quickly with the second paragraph.
Apparently, he was trying to get support from security sector, especially given his relatively liberal image.
And don’t worry, there’s more of such brutal statements, including ones here where he openly evoke the use of nuclear weapons.
Fuck, the road to Fallout 5 wasn’t meant to be an exemple.
