Species evolve, not individuals. Not to mention that having an extra appendage would cause all kinds of problems from needing more brain power to run it to biomechanical issues to simple nutritional intake requirements to keep both it and the brain functioning.

The environments humans inhabit are not terribly different from one another, at least not substantially so that someone would require another fucking limb to survive in it.

Because humans, unlike other species, are now capable of adapting our environment to suit us rather than the other way around. Without environmental pressure species evolve much more slowly. Animals supremely suited to their environments can go millions of years with little change. Just look at sharks.
However, prior to being able to change our environment to suit us, humans have gone through quite a bit of adaptation. This is still seen today in the various morphological differences between ethnicities and it's even more pronounced when you look through the fossil record at all the forerunners of modern humans.
