The Exorcist was very loosely based on real events. (i.e., a boy who was claimed to have been possessed by the devil/demons may or may not have actually been put through one or more exorcism rituals before returning to "normal.")

"The Exorcist" was based on a true story the way "Texas Chainsaw Massacre" was based on Ed Gein. In other words, someone took an idea they heard in a headline and riffed on it. "Law and Order"'s take on the Homolka/Bernardo murders was more realistic, and it was kinda wobbly.
What a fruitcake. It was debunked long ago. It's based in A BOOK, which said that the case above was based in a real fact. Now, it's not. To begin with, the case which it's said it's based the book didn't involve a girl or 360º turns of head. And anyway, it's loosely based because the author contacted the priest, who says it happened in the fifties. Go figure.
"Based on a true story" basically means that somewhere, somehow, something like this sorta happened.
I believe that Spiderman could have been based on a true story. I'm sure somewhere somebody swung from building to building by a rope.
