Cousin marriage may happen at greater rates in the Middle East and over to Pakistan than elsewhere, but you would have to prove that the level of inbreeding there is statistically meaningful outside of small populations that are relatively closed (e.g. some immigrant communities in other countries). Second-cousin marriage is close to a non-issue in terms of genetics; fertility actually maximizes with third-cousin marriage (http://en.wikipedia.org/wiki/Cousin_marriage).
Also, cousin marriage is relatively uncommon in Indonesia and Malaysia, which have very large Muslim populations.

Yes, it is totally implausible. The velocity involved is far too low. You might as well as that doing push-ups every day causes brain damage, or that wearing glasses does (I have calluses from my glasses' earpieces).
