Identified as physician and lawyer for parents to obtain a exemption for vaccinations. States is a Federal waiver and quotes disability act. Refuses to complete the actual required State ( CT ) exemption which licenses and credentials are verified via State DPH . Paperwork is not valid. Parents are paying fees to acquire this in attempt to have their school aged child not receive State required immunizations to attend school. This form appears to be a "copy and paste" as anyone's name and date of birth can be entered. Not only is this not valid there is no corroborating evidence of any medical diagnosis or disability. Physician on form: Dr. Andrew Zywiec and Christine Pazzula JD, Administrative law specialist
